Image Projection for oriented images

I have input image and expected image in below link..
Please any one tell me how to achieve this projection and if input image having other type of orientation.

actually i tried like this
I = imread('img.jpg');
udata = [0 1];
vdata = [0 1]; % input coordinate system
tform = maketform('projective',[ 0 0; 1 0; 1 1; 0 1],...
[-4 2; -8 -3; -3 -5; 6 3]);
[B,xdata,ydata] = imtransform(I,tform,'bicubic','udata',udata,...
'vdata',vdata,...
'size',size(I),...
'fill',128);
imshow(I,'XData',udata,'YData',vdata), axis on
figure, imshow(B,'XData',xdata,'YData',ydata), axis on
this gives reverse operation what i am expecting. but i don't know how to choose the rows of projective in 'maketform', in order to reconstruct original image back.
This is just example,for different image how to choose the projective row in 'maketform'.
