App Designer + Simulink

Let's say I have two Edit Field (Numeric) in App Designer, and I have a Model in Simulink that just add two numbers, how can I get the result from this Model?

I already tried to use assignin() in AppDesigner with the callback function, and run the model through sim(), but I always get error, or the value just don't pop up. Is it possible to do what I want, or the connection between Simulink and AppDesigner is just through signal problems in a certain time?

"get_param" or you can use "simout" block,then get data from matlab workspace in appdesigner
