Simulink on Raspberry Pi - see output in web browser

Hi,
I have deployed a Simulink model on a Raspberry Pi. Now, I would like to show output from the model in a web browser in real-time i.e. a graph or numeric values.
Is this possible?
I have installed an Apache web server on the Pi, so now I "simply" have to link the model to the server somehow... any clues?
