Multibody Error - Model not assembled - position violation

I have a multipbody model that involves an actuator moving linearly along a track (grey part in picture below). There are linkages that connect to the rod that moves linearly along the track (not shown yet). The flat part of the green brackets mount to ceiling structure, and allow the grey track to rotate. The rendering of the model looks as I would expect, but I keep getting an error that says "*** Model not assembled: position violation *** Resolve this issue in order to simulate the model.", and cannot figure out why.
I've attempted multiple joint configurations - pin slot joint (with and without a revolute joint), and a prismatic joint with a revolute joint). Neither work.
Can someone point out my mistake? Thanks in advance.

Hi @Ayush. Thank you for taking a look at this. Attached is the block. You can use it in both locations. Alternatively, you comment out the Loadcell subsystem and connect the Hyd Cylinder directly to the Rod Bracket.

I've been able to get this model working. I had to correct a couple of the rigid transformations to align the joints.
