simulink model using switch block

I want to make a simulink model for these given conditions in above table.

If xyz is true, then check input 2, if true, then it is automatic generated, if false then either it is automatic or human generated. then further check if it is repeated or not. then merge all this into output

You can use matlab function in simulink.

You cannot use switch block because switch case statement only checks one input. You have 4 input.
