There is a quick way to format clasification input patterns for a NN ?

For example,

I have,

I want it without many ifs to be targets for my classifer:

1=  0 0 1
1=  0 0 1
2=  0 1 1
2=  0 1 1
3=  1 1 0
3=  1 1 0
4=  1 0 1
4=  1 0 1

(Or whatever combination...)
