Using Ode in simulink

Hello, I want to use ode23s in two different states of state of flow diagram of simulink and output of my one ode solver leads to next state so can any one help with this problem I'm not getting how to do it.

You may want to start with Build a Model with a MATLAB Function in a Chart and come back with a specific question or error message if this doesn't work for you. Thanks!
