how to divide text image line by line??

i have one handwritten image ,i want to devide it line by line.

The first thing you'll have to answer: what is a line of text? Is the line between the date and the number a line of text on its own, or is it part of the date line or the number line?

What have you attempted so far? If the answer is nothing, why?

i tried to divide the image horizontally into 4 parts,

but it is not giving the expected result ,

i want to divide each line separately.

i want to segment each line .
