Data bus Signals in Simulink

As in standard data bus,there are three types of lines: data, address and control. In my simulink model, my subsystem has the three inport lines, I would like to write the three signals simultaneously to the subsystem, each data is written with its corresponding address.
For Example,
Address(16-bit): 8010 8014 8018 801C
Data(32-bit): 00000555 00000000 00000555 00000000
Control(1-bit): ___|---|____|---|____|---|____|---|
How to provide the data bus in Simulink model?

You can create Simulink Bus signals. See Using Composite Signals.
