It's in pixels. If you want in real world units, you have to determine a calibration scaling factor to convert pixels to real world units, like in my attached demo.

What can possibly be easier than multiplying by a scalar?
distanceInCm = distanceInPixels * cmPerPixel;
That's as easy as it gets. You just have to know or figure out what cmPerPixel is. That's what the demo does - it asks you to click on a know distance in the image and asks you the number of real world units for it.
If you already know from other means what cmPerPixel is, then just simply do that multiplication. Really, take another look, it's actually an easy process.
