Explanation of "Assertion failed port already connected to signal error" when generating using HDL Coder?

I am unable to share the full model but I've attached snippets of the model that is giving me issues. The error_model is the one that throws the mentioned error. The working_model does not throw an error and is able to be fully generated by HDL Coder.
